What to Bring to the Appointment

Katz and Hayne, M.D. Inc., is affiliated with Mills Peninsula Hospital and Sequoia Hospital. Please bring your insurance card, identification, and any referrals to your first visit. Also bring any previous urological laboratory work that has been performed.
